What is Home Health Care:

  Individuals who need Home Health Service delivery; providing examination, examination, analysis, treatment, medical care and rehabilitation at home and in the family environment by an expert staff; Providing social and psychological support services to these people and family members together as a whole, following up the treatment of the diagnosed patient, providing the necessary training, and establishing, referral and administration of home health services to be provided by the Ministry and its affiliates. It is the health service that ensures coordination between institutions and organizations.

How to Apply for Home Health Care:

Patients or their relatives who want to benefit from Home Health Services can apply by calling the Home Health Services National Communication Center at 444 3 833.

Who can apply to Home Health Service:

   Individuals who have difficulty in accessing health care services due to their illness and/or old age that impair their quality of life and who need health care at home and in their family environment can apply.

Patient Admission and Examination Process
           Patients who have applied to home health services through the National Contact Center for Home Health Services; Aydın Province Home Health Services Coordination Center is called within 15 minutes following the application and assigned to the closest team according to the residence address and the most appropriate team according to the diagnosis and service demand of the patient.

The team reaches the patient within 24 hours. During the home visit, the patient is evaluated and it is decided whether the patient will receive home health care. The patient and his relatives are informed. If the patient is eligible to receive home health services, the team is informed about the progress. The person to be provided with home health care services, their parent or guardian will have the home health services consent document read and signed at the first examination.

Follow-up of the Patient: In line with the diagnosis and treatment plan determined for the patient to be given home health care, visits are made at certain intervals within the framework of the planning made in line with the needs of the patient in the home environment.

Transfer Process of the Patient: The patient in need of transplantation is taken from his home by a suitable vehicle and transferred to the hospital where he will go, and he is left home after the examination or treatment.

Termination of Service: The service is terminated if the person who receives or continues to receive home health care does not want to receive home health care service voluntarily, the relevant physician decides that the need for home health care is not needed, the treatment becomes sustainable without the health personnel, the patient recovers, and the patient dies.

NAZİLLİ State Hospital Home Health Services Unit provides home health and transport services to an average of 1347 patients per month, with 2 specialist physicians, 2 general practitioners, 18 health personnel, 1 physiotherapist and 3 transport ambulances and 5 service vehicles that provide access to the service.


NAZİLLİ HOME HEALTH SERVICES UNIT
  A new Palliative Patient Follow-up Unit was established and started to serve in order to provide better on-site and effective service to our patients who entered the palliative process and were followed up in the inpatient service and then went to the stage of being followed up at home as palliative patients. Our team will closely monitor approximately 500 fully dependent patients in terms of palliative necessity .
