What is Home Health Care:
It is a health service that covers the provision of examination, examination, analysis, treatment, medical care and rehabilitation at home and in the family environment by a staff specialized in this field; the provision of social and psychological support services to these individuals and their family members as a whole, the follow-up and implementation of the treatment arranged for the diagnosed patient, the provision of necessary trainings, as well as the establishment, referral and management of home health services to be provided by the Ministry and its affiliated organizations and the coordination between the relevant institutions and organizations.
How to Apply for Home Health Service:
Patients or relatives of patients who wish to benefit from Home Health Services can apply by calling the National Contact Center for Home Health Services at 444 3 833.
Who can apply for Home Health Service:
Individuals who have difficulty in accessing health services due to illness and/or old age that impairs quality of life and who need health services at home and in the family environment may apply.

Patient Admission and Examination Process:
Patients who have applied for home health services through the National Communication Center for Home Health Services are called by the Aydın Provincial Home Health Services Coordination Center within 15 minutes following the application and assigned to the closest team according to the residence address and the most appropriate team according to the diagnosis and service request of the patient.
The team reaches the patient within 24 hours. During the home visit, the patient is evaluated and it is decided whether the patient will receive home healthcare services. The patient and his/her relatives are informed. If the patient is eligible to receive home health care, the team informs the patient about the progress. The person, parent or guardian of the person to be provided with home health services is made to read and sign the consent document for home health services during the first examination.
Patient Follow-up: In line with the diagnosis and treatment plan determined for the patient to be provided with home health care services, visits are made at regular intervals within the framework of the planning made in line with the needs of the patient in the home environment.
Patient's Transplant Process: Patients in need of transportation are picked up from their homes with the appropriate vehicle, transported to the hospital they will go to, and left at home after the examination or treatment.
Termination of Service: The service is terminated if the person receiving or continuing to receive home health care services does not want to receive home health care services voluntarily, the relevant physician decides that the need for home health care services is no longer necessary, the treatment becomes sustainable without health personnel, recovery and death of the patient.
NAZILLI State Hospital Home Health Services Our unit provides home health and transportation services to an average of 1347 patients per month with 2 specialist physicians, 2 general practitioners, 18 health personnel, 1 physiotherapist and 3 transport ambulances and 5 service vehicles.
NAZILLI HOME HEALTH SERVICES UNIT
In order to provide better on-site and effective service to our patients who enter the palliative process and are followed up in the inpatient ward and then followed up at home as a palliative patient, a new Palliative Patient Follow-up Unit has been established and started to serve. Our team will closely monitor approximately 500 fully dependent patients in terms of palliative necessity.
